Set the Date – Vaccinate!

The Florida Department of Health in Collier County (DOH-Collier) encourages all parents and caregivers to check the immunization status and school requirements for their children.
It is critical that all students have their required immunizations. Immunization documentation on a DOH Form 680, is required for all students in Kindergarten through 12th grade. DOH-Collier recommends parents to call their child’s physician for an appointment or come to the health department. Waiting time at DOH-Collier locations is minimal at this point, but the longer parents delay, the greater the likelihood of having to wait.
In addition, on Friday, August 10, 2018 DOH-Collier will be hosting a “Back-To-School Health Fair” to encourage parents to bring their children to receive their immunizations. The Health Fair will be held at DOH-Collier’s Naples and Immokalee sites.
Free immunizations for children through 18 years of age are available at the following Collier County Health Department locations:
* Building H, 3339 East Tamiami Trail, Naples, Monday through Friday from 8am-3pm
* 419 1st Street North, Immokalee, Monday through Friday from 8am-4pm
Appointments are not necessary. For questions, please call 252-8595 (Naples) or 252-7300 (Immokalee). Parents are asked to bring their child’s shot record when they come for immunizations.
