Services 

The Collier County Health Department serves as the county's vital records office for birth and death certificates. Death certificates are issued by the Health Department for the year 2009 to the present for those individuals who passed away in the state of Florida. Death certificates for other years must be requested from Jacksonville.

Appointments are not required. All services are provided on a walk-in basis.

Eligibilty Requirements 

Death certificates, with cause of death, can be issued to immediate family members of the deceased or to a licensed funeral director.

Cause of death is confidential information and can only be issued to immediate family members, which FS 382.025 defines as "spouse or parent, or to the registrant's child, grandchild, or sibling, if of legal age, or to any family member who provides a will, insurance policy, or other document that demonstrates the family member's interest in the estate, or to any person who provides documentation that he or she is acting on behalf of them." Cause of death cannot be shown for anything that will be filed as a matter of public record, i.e., real estate transfers; automobile transfers, recording of wills, etc.

Fees or Payment 

Death certificates issued by the Health Department cost $10.00 each. We accept cash, check, money order, or Visa and MasterCard towards payment.

Application Form 

Please download and print out an application for Collier County Death Record and mail with payment of $10.00 for each certificate, as well as a self-addressed, stamped envelope (legal-sized if ordering more than 5 certified copies), and mail to the post office box above.
