Eligibility Requirements

Adults 19 and older. Please bring documentation of past vaccines if available.

Fees or Payment

  • $40 nursing assessment fee unless client is scheduled for immigration physical with the health department
  • Individual vaccines prices will apply
  • Medicare, Medicaid, private insurance, private pay. We will bill your insurance
  • Co-payments and payments for vaccines not covered by insurance are due at time of service
  • Vaccine patient assistance programs are available for some vaccines for those who meet financial eligibility. Applications can be processed during your visit.
