The primary goal of the Chronic Disease Workgroup is to increase the impact of evidence-based programs that promote healthy choices and reduce chronic disease risk factors. The five strategies and lead agencies are shown below:

  • Offer evidence-based nutrition education throughout Collier County. (Blue Zones Project Southwest Florida)
  • Identify children and families at risk of food insecurity. (American Heart Association)
  • Increase participation in the Togetherhood Initiative Pathway to Wellness programs for Collier County youth. (Togetherhood Initiative)
  • Increase health awareness messaging in the community (DOH-Collier)
  • Encourage local municipalities to adopt tobacco-free beaches and parks policies. (Tobacco Free Collier)
