The primary goal of the Chronic Disease Workgroup is to increase the impact of evidence-based programs that promote healthy choices and reduce chronic disease risk factors. The five strategies and lead agencies are shown below:
- Offer evidence-based nutrition education throughout Collier County (Blue Zones Project Southwest Florida)
- Identify children and families at risk of food insecurity (American Heart Association)
- Increase participation in the Togetherhood Initiative Pathway to Wellness programs for Collier County youth (Togetherhood Initiative)
- Increase health awareness messaging in the community (DOH-Collier)
- Encourage local municipalities to adopt tobacco-free beaches and parks policies (Tobacco Free Collier)
