Services 

The Collier County Health Department serves as the county's vital records office for birth and death certificates. The Health Department can issue birth certificates for the years 1930 to the present for births within the State of Florida.

Birth certificates are available from the Collier County Health Department for ALL Florida counties. Birth Certificates are needed for:

  • A Passport
  • Admission to public school
  • To obtain a driver's license
  • A work permit
  • The right to vote
  • Eligibility for retirement
  • Social Security benefits
  • To prove citizenship
  • To obtain public assistance for dependent children

Monday - Friday 8 am to 4:30 pm

Appointments are not required. All services are provided on a walk-in basis.

Eligibilty Requirements 

Birth certificates can be issued to:

  • Parents
  • An individual 18 years of age or older
  • To legal guardians with proof of legal custody with proper identification

Types of identification accepted are: State I.D. Card, Driver's License, Passport, or Military I.D.

Fees or Payment 

Birth certificates issued by the Health Department cost $12.00 for all Florida Births. A second certificate costs $8.00 if issued at the same time as the first certificate. We accept cash, check, money order, or Visa and MasterCard towards payment.

Application Form 

Please download and print out an application for Florida Birth Record and mail it in with payment to the Post Office box above. Note: Please submit a copy of your ID if you are 18 or older along with your application and payment. If you are not 18 or older, submit a copy of your parent/guardian's ID.
