To report a PUBLIC HEALTH EMERGENCY 24 HOURS A DAY, 7 DAYS A WEEK

Health care providers play a key role in the public health department’s efforts to control communicable diseases. Health care practitioners are required to report notifiable diseases or conditions in the State of Florida (mandated under Florida Statute 381.0031 and Rule 64D-3, Florida Administrative Code [F.A.C.]).
