Services are provided for adults age 18 and older. Please bring forms and specific requirements of your college or place of employment to your first visit.
Self pay or prior insurance authorization for medical examination and completion of forms. Immunizations and communicable disease screening performed based on the school or employment requirements. Individual fees will apply.
