- Case management is the first step to accessing medical and dental care. This program includes case management, treatment adherence support, medication, housing assistance, and referrals as needed.

- Proof of insurance
AIDS Drug Assistance Program (ADAP)

Housing Opportunities for Persons with AIDS (HOPWA)
Services
- Provides temporary housing assistance to eligible individuals living with HIV/AIDS
- Services include transitional housing, assistance with rent, mortgage, and utilities
Requirements
- The program allows clients to be at 80% or less of the median income
- Current Ryan White Notice of Eligibility

Services
- Medical care and treatment for people living with HIV/AIDS
- Laboratory Services
- Medical case management/care coordination
Requirements
Clients must register with HIV case management by calling 239-252-8593 prior to scheduling an appointment for medical services.
HIV, STD, and Hepatitis Counseling and Testing
Services
- Rapid HIV testing with test result in 20 minutes
- Syphilis, chlamydia, gonorrhea, and herpes testing
- Hepatitis screenings and vaccines to high risk populations
